Introducing Queenscourt’s TRANSFORM Team
TRANSFORM Team: Works across hospitals,
community services and care homes, to support
clinical sta昀昀 and ensure that people approaching
the end of life receive consistent, compassionate
and high-quality care, wherever they are supported.
The team provides education, guidance and practical
support for clinical sta昀昀 caring for people in the 昀椀nal
months and days of life.
Bringing end-of-life care training to life
As part of their education programme, the team has
developed The Travelling Street – a portable, tabletop
simulation that helps healthcare professionals explore
the realities of palliative and end-of-life care.
Using miniature care environments, wooden peg dolls
and patient stories, this interactive ‘escape room’ guides
participants through a 昀椀ctional resident’s end-of-life
journey. Working together, they solve challenges linked
to the Five Priorities for Care, exploring communication,
decision-making and co-ordinated care.
The practical format encourages teamwork, discussion
and re昀氀ection while building con昀椀dence in supporting
patients and families.
Over seven months, The Travelling Street was delivered
24 times across hospital, hospice and care home
settings, reaching 154 participants, including nurses,
